Weight Management program

This is not about pills or supplements or trending diets. This is about developing a personalized plan that will help you build food and exercise habits to keep you healthy and in shape for decades to come. By working with Dr. Jacob on your weight loss, a personalized plan will be developed that includes physician supervision, fitness coaching, nutrition coaching, bloodwork/labs, and (if appropriate) medications.

Weight Management

This 9-12 month program specifically focuses on weight management. You will receive a personalized program to set goals and achieve sustainable lifestyle changes that address the four pillars of health: nutrition, movement, sleep, and mental health. The program is intentionally designed to facilitate lasting change using the principles of habit development and psychology.

Program Features:

  • Learn what is the necessary balance of limitless options out there. Learn how to focus on mindful eating practices to improve digestion and overall well-being.

  • Learn the optimal ratio of various exercises for your body type.

  • Take a deep dive into your sleep habits and learn how to optimize for overall health and well-being. If needed, we will make a referral to a sleep center if we find concerning symptoms. 

  • Learn stress-reduction techniques to improve well-being.

  • Learn techniques for fostering emotional resilience and mental well-being. If needed, we will make a referral to a professional psychologist or psychiatrist.

  • Additional medications may be recommended which can range from $249 to $499 monthly based on dosage (discounted for concierge members).

  • Dr. Jacob reviews your existing labs or orders new labwork to get a comprehensive evaluation of your current health. This allows evaluation for alternate causes of weight gain as well.

  • If you are a part of the Ascend Wellness MD membership program, included in your promotional rate of $45 (normally $65) you are entitled to:

    - 1-2 written workouts a month

    - minimum of 3 check-ins a week

    - one 30-minute virtual session

  • - written workout $25

    - 30 min in person session $60

    - 30 min virtual $45

    - 45 min small group session, 2 - 4 adults - $125

  • - 30 min in-person session - $45

    - 30 min virtual session - $35

    - 45 min small group session of 2 - 4 adultsm- $100
